Anatomic topographic features of pseudophakic eyes with pseudoexfoliative syndrome by means of ultrasound biomicroscopy

Purpose
To study anatomic topographic features of pseudophakic eyes after phacoemulsification with intraocular lens implantation in cases of cataract with pseudoexfoliative syndrome (PEX) by means of ultrasound biomicroscopy (UBM).
Material and methods
UBM was performed in 109 eyes of 93 patients. The average age was 71.72 years (55-83). The main group included 70 patients (74 eyes) with expressive clinic features of PEX: flakes of pseudoexfoliative material was found on atrophic edge of iris and lens. The comparative group included 23 patients (35 eyes ) without clinic features of PEX. UBM “SONOMED” (USA) examinations were performed and following parameters were measured: anterior chamber depth (ACD), «trabecula-iris» distance (mm), thickness of the ciliary body (mm), length of ciliary zonule fibers (mm).
Results
The presence of PEX inclusions flakes were found on the iris, lens, ciliary body and associated with zonular weakness and appearance of ruptures in some zonular fibers by UBM.
The depth of anterior chamber was deeper in pseudophakic eyes and depended on initial clinical refraction. The increase of trabecula-iris distance 500 ?m (from 0.1 to 0.4) were revealed in all cases, and reflected an anterior-chamber angle enlargement. The decrease of visualized ciliary zonule fibers was found in all groups of the study. The increase of thickness of the cilliary body took place as well.
Conclusions
The obtained features of anatomic topographic ratio of ocular structures specify an expansion of functional space for structures of iridocilliary zone after cataract phacoemulsification. Cataract phacoemulsification eliminates asymmetry in the topography of irido-cilliary zone structures. Expansion of functional area for structures of eye anterior segment after removal of cataract lens becomes apparent by increasing of the anterior chamber depth and the anterior chamber angle that is more pronounced in hyperopic eyes.
