Abstract
Topical antimicrobials for the prophylaxis of infectious complications of intravitreal injections

Disinfection of the injection site and the accompanying peri-injection courses of antibiotic therapy are the most important measures for preventing infectious complications following intravitreal injections (IVI), among which endophthalmitis has the greatest potential threat to the patient.
In 65-70% of the 40 patients who were to be injected, coagulase-negative staphylococci, mainly Staphylococcus epidermidis, as well as S. aureus and few Gram-negative bacteria, were isolated from the conjunctiva. Among the isolated coagulase-negative staphylococci, 40-50% were multidrug-resistant. Treatment of the eyelid skin and the area around the eyes with a 10% solution of iodopyrone and irrigation of the conjunctiva for 30 seconds with 2.0 ml of 5% povidone-iodine together with preventive courses of antibacterial drugs «Tobrex» or «Vitabact» 3 days before and within 5 days after the injection resulted in complete elimination of conjunctival microflora isolated prior to treatment.
In 10% of patients of the «Tobrex group» and in 20% of patients of the «Vitabact group» secondary infection of the conjunctiva was detected 1 day after the end of the treatment. The bactericidal effect of picloxydinedihydrochloride in Vitabact eye drops is comparable to that of the antibiotic drug tobramycin with a single course accompaning IVI. Vitabact can be a credible alternative to antibiotics for the prevention of infectious complications during monthly repeated courses of IVI, when the risk of developing of ocular microflora antibiotic resistance increases.
