Purpose. To compare clinical and functional results of surgical treatment of big macular ruptures (МR) with the use of developed method of the formation of internal border-line membrane fragment to close macular holes and standard surgery.
Material and methods. According to the developed method, 78 patients were operated (main group). Surgical approach is described. 44 patients (control group) were operated by classical method of maculorhexis, with rupture edges approximation by way of vacuum aspiration and gas tamponade of vitreal cavity.
Results. After the surgery in the main group it was found a statistically significant visual acuity growth compared to the control group. Structural data analysis in the main group revealed a statistically significant growth of frequency of full MR closure, as well as subsequent decrease of the width or defects disappearance on the level of ellipsoid area of photoreceptors compared to the control one.
Conclusion. The developed method of formation of internal border-line membrane fragment to close macular holes is effective and allows achieving more high morpho-functional results in surgical treatment of big macular ruptures compared to the standard approach, which include the conduction of classic maculorhexis and rupture edges approximation by way of vacuum aspiration.
Key words: idiopathic macular rupture, maculorhexis, vitrectomy, internal border-line membrane.
