FA-support in complex treatment of proliferative diabetic rethyopathy

Purpose. To evaluate the effectiveness of delayed precision transpupillary retinal laser coagulation (LKS) under the control of fluorescent angiography (FA) in the complex treatment of proliferative diabetic retinopathy (PDR) after vitrectomy.
Material and methods. The study included five patients (3 women, 2 men) with DR, type 2 diabetes mellitus, insulin-dependent, with advanced stage of PDR. Patients underwent a 3-port vitrectomy with the use of 25g systems. One month after the surgical treatment, all patients underwent wide-angle FA and performed LKS.
Results. After 1 month, in all cases, according to the data of the wide-field FA, extensive zones of nonperfusion, multiple intra-retinal microanueresms were revealed. LKS to all patients was carried out strictly in the projection of vascular changes. After 3 months, according to the FA, new zones of non-perfusion, areas of neovascularization were not revealed.
Conclusions. The use of FA a month after vitrectomy makes it possible to diagnose pathological changes and determine the area of the ischemic retina. It allows to carry out LKS in full, excluding zones of intact tissue that promotes preservation of visual functions and minimization of complications.
