Surgical approach for treating large and giant macular rupture

Purpose.To analyze the results of surgical treatment of patients with big and giant macular raptures (over 1000 microns).
Material and methods. Surgical approach involved micro invasive 25G+ vitro ectomy, peeling of the retinal internal limiting membrane, converging of rupture edges by silicone-point cannula with subsequent introduction of several drops of venous autoblood to the remaining slit-like space between rupture edges. The manipulations with autoblood and converging of rupture edges were performed in moderate hypotension conditions. The surgery was completed by “drying” the vitreous body and tamping by silicone oil. 37 patients with big macular raptures were operated under this method.
Results. In the early postoperative period 21 patients (57%) were diagnosed total blocking of macular rapture on the 1st -4th day. In the long run up to 36 months, 16 patients (43%) were diagnosed a continuous closure of the rapture.
Conclusion. This method improves the anatomical and functional results of the treatment of patients with large and giant macular raptures.
