Clinical results of refractive lensectomy in patients with moderate and high degree hypermetropia

Hypermetropia is a common refractive anomaly affecting a significant part of the population. In modern practice, phacoemulsification followed by implantation of an intraocular lens (IOL) is becoming increasingly popular as a method of correcting farsightedness due to the development of microinvasive technologies and new IOL models. Refractive lensectomy research is mainly focused on age groups over 45–50 years old, which requires a study of efficacy and safety in young and middle-aged patients.
Purpose. To evaluate the clinical efficacy and safety of refractive lensectomy followed by multifocal IOL implantation in young and middle-aged patients suffering from moderate and high degree hypermetropia.
Material and methods. A retrospective clinical trial was conducted, where the data of 20 patients (40 eyes) with moderate and high-grade hypermetropia, aged from 23 to 50 years, were analyzed. All patients underwent refractive lensectomy with multifocal IOL implantation (AcrySof IQ PanOptix or Alcon Clareon PanOptix).
Results. Preoperative hypermetropic refraction averaged 4.75±1.5 D (2.25–8.5 D), NCOSIS — 0.2±0.19 (0.03–0.8), MKOZ — 0.8±0.23 (0.25–1.0). After surgery, the refractive index averaged 0.08±0.28 D, visual acuity without correction increased from 0.2 to 0.8 on average. The planned emmetropia was achieved in 87.5% of the eyes.
Conclusion. A retrospective study confirms the efficacy and safety of phacoemulsification of the crystalline lens with the implantation of a multifocal IOL for the correction of moderate and high degree hypermetropia. The safety of the procedure is confirmed by the absence of cases of a decrease in the MCI by more than one line.
