Purpose. To analyze the organization and formation of the vitreoretinal surgery department to provide specialized ophthalmic surgical care to patients in the Far Eastern Federal District (FEFD).
Material and methods. The staff of the department is 5 ophthalmic surgeons. Operating room equipment: ZEISS OPMI Lumera 700 microscopes, including those with an integrated dynamic optical coherence tomography system (Carl Zeiss, Germany), latest generation ophthalmosurgical units EVA (DORC, the Netherlands) and Constellation (Alcon, USA), NGENUITY 3D visualization system (Alcon, USA).
Results. The difficulties of mastering the technology of performing vitreoretinal surgery by novice surgeons are described. For 2019–2021 8042 surgical operations were performed: for retinal detachment — 23.3%, proliferative diabetic retinopathy — 15.5%, idiopathic macular holes — 12.5%, hemophthalmos — 8.3%, epiretinal membranes — 5.2%, etc. 22.8% of operations are represented by the removal of silicone oil from the vitreous cavity. Retinal detachment (RD) surgery was performed on 1865 eyes, while 1412 eyes were with rhegmatogenous RD, 453 eyes were with traction RD. In 1223 eyes with rhegmatogenous RD, endovitreal interventions were performed, which ended with silicone oil tamponade. 1238 eyes were operated for proliferative diabetic retinopathy (subtotal vitrectomy followed by silicone oil tamponade). In surgery for idiopathic macular holes (1009 eyes), the most commonly used technology was membrane peeling with pneumoretinopexy, silicone oil tamponade, as well as the method of reverse flap of the internal limiting membrane. Removal of idiopathic epiretinal membranes was performed in 418 eyes. The demand for vitreoretinal surgery in FEFD patients is high. Despite the rather large volumes of performed surgery, up to 16–18 patients referred from other medical institutions to the department every week with various pathologies. Of these, approximately 8–10 people have urgent indications for surgery.
Conclusion. The volume of surgical interventions in the specialized department of vitreoretinal surgery for three years amounted to 8042 operations performed in 6215 patients. The analysis showed a high demand for vitreoretinal surgery in patients of the FEFD.
