Cicatrizing conjunctival pemphigoid (PKK) is one of the most rare and severe autoimmune diseases of the eye with a chronic course. The article presents a clinical observation of a 69-year-old patient with a complete fusion of the palpebral fissure (outcome of the disease in the form of complete ankyloblepharone) of one eye and pronounced symblepharone and shortening of the conjunctival vaults and limitation of eye movement, trichiasis and torsion of the eyelids, as well as corneal conjunctival xerosis of severe degree, vascularization clouding of the cornea of the paired eye.
The authors recommend that ophthalmologists pay attention to long-lasting and refractory to conventional treatment conjunctivitis (usually initially evaluated as bacterial) in elderly patients, especially those beginning after previous erosive changes of the oral mucosa and nasopharynx (which is often diagnosed as respectively stomatitis and rhinosinusitis) and complicating over time, cicatricial changes and conjunctival adhesions (at the beginning of the disease – in the lower vault). Attention is focused on the fact that only timely diagnostics with involvement and alertness regarding this disease by doctors of other specialties (otolaryngologists, dentists, dermatologists, etc.), started early and intensive, not only local, but necessarily systemic treatment with immunosuppressants (corticosteroids, cytostatics) in combination with symptomatic therapy can contribute to long-term remission of this disease and preservation of visual functions.
