Transscleral laser treatment of end-stage glaucoma in the micropulse mode: a pilot study
Abstract
Transscleral laser treatment of end-stage glaucoma in the micropulse mode: a pilot study.
Objective. To evaluate the results of transscleral laser treatment of end-stage glaucoma in the micropulse mode conducted within the clinical approbation of the Cyclo G6 Glaucoma laser system (IRIDEX, USA).
Material and methods. We conducted a comparative evaluation of the treatment of terminal-phase glaucoma by contact diode-laser cyclophotocoagulation and transscleral microimpulsive cyclophotocougulation.
Results. All operations were performed without complications. After microimpulse cyclophobic coagulation there is an effective deprivation of pain syndrome against a background of achieving a stable hypotensive effect. The number of antihypertensive drugs in the patients of the main group decreased on average from 3.0 (before surgery) to 1.53 after surgery (after 4 months). The number of hypotensive drops in the control group was 3.0 before surgery, after surgery, 2.1 (after 4months).
Conclusion. The data of the conducted study show that the transscleral microimpulsive cyclophotocougulation is an atraumatic, safe method of treatment of severe forms of glaucoma, it allows to eliminate pain syndrome and achieve a predictable effect with minimal risk of postoperative complications. The results of repeated ultrasonic biomicroscopy studies convincingly testify to the preservation of the anatomical and topographic correlations of the ciliary body against the background of moderate hydration in the early postoperative period, which indicates the safety of the technique in terms of the risk of developing severe complications such as hypotension and subatrophy of the eye.
Key words: terminal stage of glaucoma, transscleral microimpulse cyclophotocoagulation, contact diode-laser cyclophotocoagulation, ciliary body, ultrasonic biomicroscopy.
