Purpose. To present a rare clinical case of Purcher's retinopathy after a closed head injury.
Material and methods. Patient R., 56 years old, after a traumatic brain injury with complaints of a sharp decrease in vision in both eyes. Performed optical coherence tomography with the function of angiography to identify specific markers of the disease, diagnosed with Purcher's retinopathy.
Results. The patient was prescribed treatment with glucocorticosteroids, non-steroidal anti-inflammatory drugs and diuretics. Angio- and neuroprotective therapy was also carried out. A week later, the patient noted a positive trend from the treatment. Vision increased to VOD = 0.5 does not correct, VOS = 0.5 does not correct. On the fundus, complete resorption of hemorrhages and Purcher's spots. On the images of optical coherence tomography with the function of angiography, there are no areas of ischemia, the edema was stopped.
Conclusion. The presented clinical case of Purcher's retinopathy proves the possibility of developing acute vascular pathology in the fundus, against the background of a closed craniocerebral injury, without direct impact on the eyeball. Optical coherence tomography with angiography function helps in making the correct diagnosis with the identification of all specific markers of the disease, such as ischemic edema, Purcher's spots and retinal hemorrhages.
